I understand that if I need to cancel my appointment out of courtesy I will try to give at least 24 hours notice in advance.

I understand that Klara (Guided Bloom) is not a licensed therapist, psychologist or health care practitioner and offers EFT and Matrix Reimprinting sessions only. I am aware that Klara (Guided Bloom) does not diagnose or treat illness or disease, and does not prescribe medications. I agree not to discontinue or change any medications I am taking while working with Klara (Guided Bloom) without consulting my doctor. I accept that no guarantee of a cure can be given for any presenting issue(s). And I agree to release Klara (Guided Bloom) from all liabilities and claims for any causes, past, present and future.

I understand that all information I share with Klara (Guided Bloom) is confidential and any information will not be released without written consent, with the following exceptions: 1) When there is imminent risk or danger to myself or another person 2) When there is suspicion that a child, elder or dependant is being sexually or physically abused or at risk of such abuse 3) When a valid court order is issued for session records.

I agree to take complete responsibility for my own comfort, health and well-being while working with Klara (Guided Bloom).
